ADDITIONAL INFORMATION:

Registration includes all meals and materials for the Member Renewal Retreat, but excludes hotel stay. Conference participation is limited to the first 60 registrants. Please submit this form and payment promptly to ensure your place at the Renewal Retreat Weekend. The deadline for advanced registration is May 23, 2006. Participation is limited to WDS members and pending members only.

CANCELLATION POLICY:

Refunds, less $25 administration fee, will be available for cancellation prior to May 23, 2006. After that date, no refunds will be given.

There is no provision for partial registration fees or refunds for activities not attended. For further information, contact the WDS office at 877-WDS-ROSE (937-7673)
